Screening of Hosts

Background Checks

All prospective Host Home contractors and all family members living in the home must

undergo comprehensive criminal background checks, including checks for child and

dependent adult abuse/neglect, sex offender registry, and OIG registries. Any “hits” on

background checks of host home providers or family members living in the home are

reviewed by the Department of Human Services to determine host home contractor

eligibility.

Interviews

Agency staff conduct structured interviews to assess contractor suitability,

communication skills, and caregiving experience.

Documentation Requirements

*Completed application forms

* Background checks

* Proof of residency and identification

* Assessment of skills and temperament necessary to provide care

* References from previous employers and/or personal contacts

* Verification of home ownership or rental agreement

* Initial Safety Assessment of home and reassessment conducted no less than

quarterly.

Quality First

Host Home Providers must:

  • Know each individual’s baseline condition
  • Complete required training (CPR, First Aid, medication manager, behavioral supports)
  • Maintain open communication with the care team
  • Ensure timely and accurate reporting

Expert Care

  • 24/7 live support from an approved Agency Support Specialist
  • Regular feedback sessions with contractors
  • Surveys to assess satisfaction and identify areas for improvement

Initial Safety and Health Checks

Prior to placement, agency staff conduct comprehensive home inspections to verify

safety, accessibility, and health standards. Areas assessed include fire safety,

sanitation, accessibility, and medication storage.

Regular Assessments

  • Scheduled home visits at least quarterly
  • Unannounced checks as needed
  • Documentation of findings and corrective actions
